West Valley High School : Sports

Activities Information Form: Athletes will obtain a West Valley School District Activities Information Form and make certain that all items are PROPERLY completed. Turning to Building Athletic Director/Main Office or Head Coach.
Physical exam procedures for athletes: the physical examination must certify that the athletes physical condition is adequate to participate in interscholastic athletics (see item 18.13.0 of student standards for interscholastic athletic eligibility). Turn in to building athletic director/main office or head coach (See Appendix B for official WIAA Physical form).
Parent/athlete sport specific consent. Turn in to building athletic director/main office/head coach.
Concussion Management (Lystedt Law) Information Form signed/dated by parent and athlete (yearly)
Have a valid ASB card.
Pay the pay-to-participate fee in the JH/HS/MS office by the posted deadline.

In order to participate, a prospective student/athlete must be on the team roster by the end of the 3rd week (the exception would be a transfer student in a non-cut sport, who has met the eligibility criteria at his/her former school in that sport)
